Incident Reporting and Emergency Preparation
Please report any Incidents to your supervisor or Faculty/PI.
Incident Reporting
An Incident that occurs and results in an injury to an ASU paid employee (faculty, researchers, staff, student worker) or unpaid student or visitor must be reported.
Any Incident resulting in ASU property damage(s) exceeding a cost of $500 must be reported.
EHS must be notified of all incidents that fall under the above scenarios.
Personal injury, Property damage, and/or Material Loss
- Notify your Supervisor/Faculty/PI and FSE EDO IaST Lab Manager.
- Complete Incident Reporting Form (on-line) within 24 to 48 hours of the Incident.
- Contact Engineering Technical Services (ETS) for Equipment repairs.
- Contact FDM Service Request (FACMAN) for Facilities repairs.

Emergency Preparedness
To create a safe work environment or special event it’s important to consider the risk in all activities whether planned or unplanned. As a way to be safety minded during an emergency you should:
Anticipate potential risk
You should proactively evaluate a process or event for possible incident factors such as infrequent occurrence, improper or insufficient training, time constraints that may cause rushing, etc.
Respond to emergency
An emergency response should always begin with following instructions given when calling ASUPD, Tempe dispatch, or information found in the Emergency Response Guide.
Once the event is concluded, you should file an Incident Report with ASU EHS.
Evaluate afterwards
Please forward a copy of your communication to [email protected]. FSE DO Infrastructure and Safety Team will follow up with you for an Incident Review to ensure all hazards have been eliminated and we address the safety concern that lead to the Incident.
There are no resulting disciplinary actions taken from reporting an Incident. We are here to assist with ensuring safe work and research environment for our FSE and ASU Community.
Building Emergency Plans
Familiarize yourself with the emergency protocols of the building and also infrastructure details such as location of fire extinguishers, pull stations, and emergency exits. An Emergency Evacuation plan has been created for each building. During the Fire Safety Training class, you should have received the Fire Evacuation plan for your building, that identifies the Primary and Secondary Evacuation locations.
